In-home Injury Prevention
If you or someone you support is aged 65+, or 55+ with a high risk of falling, you may qualify for a free in-home safety assessment and equipment installation.
Falls are the most common cause of injury, especially for older adults, and can lead to serious injury, hospitalisation or loss of independence.
ACC’s In-Home Injury Prevention programme aims to help reduce preventable injuries by making homes safer for people at risk of falling.
The programme is funded by ACC, and delivered in Thames-Coromandel by Focus on Potential clinicians. Participation is completely voluntary and will not affect any current or future ACC claims.
Who can apply?
You may be eligible if:
- You live in the Thames-Coromandel District, and
- You are 65 years or older, or 55+ with a high risk of falls, and
- You have had a fall or are at risk of falling, and
- You expect to live in your home for at least three years. This can be
- your own home, or
- community or social housing.

What will I receive?
Eligible participants will receive:
- a free in-home safety assessment with a registered clinician
- up to two grab rails installed by a registered builder at no cost to you
- a free non-slip safety mat.
How does the process work?
- To request an assessment, complete the form below.
- We contact you to confirm your eligibility.
- If you are eligible, we arrange a convenient time for your assessment.
- A Focus on Potential clinician will visit and complete an in-home safety assessment with you.
- If recommended, we will arrange for installation of safety equipment by a registered builder at no cost to you.
